Better Days at Home

Better Days at Home is a unique Canopy Health palliative care program designed for people, at any age, living with advanced illnesses. This program uses sound medical care with other physical and emotional comfort care treatments to improve the quality of life for patients and their families who are living with advanced illnesses. When someone has a serious medical condition, their whole family goes through the challenges of seeing their loved ones’ health decline every day. Patients and family members can get care at home and pay the usual copay for the services they receive.

In the Better Days at Home program, a team of trained nurses, social workers, and doctors work together to provide relief of stress and care to manage the patient’s symptoms. Patients still get healing treatment from their regular doctor to make sure they receive the right care and avoid unnecessary hospital visits. The care team may also include chaplains and other providers who can offer spiritual support. The care provided is tailored to each person’s individual needs and goals.Â
