Alliance Referral Program
The Alliance Referral Program allows members to see any specialist in the
Canopy Health network for medically necessary specialty care as well as access
Ancillary Providers across Canopy Health’s network. A PCP referral and
administrative authorization from the PCP’s IPA/Medical Group are required for
tracking purposes. OB/GYN services require an administrative referral to
facilitate claims payments only. For more information on the Alliance Referral
Program, please call (415) 712-1020 Monday-Friday 8:30 am – 5:00 pm PST (closed
holidays).
The one exception is lab services. Members must be referred to their Home IPA/Medical Group’s contracted lab.Â

Filing A Claim
Providers are encouraged to file claims electronically whenever possible. Submitted claims should provide all required information; those submitted with missing data may result in a delay in processing or denial.Â
All Canopy Health facility claims are processed by Conifer Value-Based Care. Professional claims will continue to be processed by the participating Medical Groups and/or their respective vendors.
