The Santa Clara County IPA (SCCIPA) offers high-quality, coordinated medical care to its patients. Now it is joining Canopy Health’s impressive alliance of Bay Area hospitals, physicians, and care centers. 

Access to Diverse Medical Providers Throughout Santa Clara County

SCCIPA is the largest individual provider association in Santa Clara County. Its network includes more than 800 primary care physicians, hospitalists, and specialists. SCCIPA partners with Good Samaritan Hospital, San Jose and the Regional Medical Center of San Jose, which are also joining the Canopy Health alliance. The Association’s physicians and staff speak 33 languages and have diverse backgrounds, reflecting the diversity of their community.

Coordinated Medical Care Within SCCIPA and the Canopy Health Alliance

SCCIPA was a pioneer in coordinated, accountable health care in the Bay Area. In the 1980’s, a group of independent physicians realized they needed to restructure their practices to compete with increasingly larger medical groups. Together, they founded SCCIPA in 1986.

All SCCIPA patients receive coordinated care from a primary care physician (PCP). This doctor manages all of their chronic conditions and focuses on preventive care. When referrals are necessary, the PCP can send them to an in-network specialist. Labs, diagnostic testing, and other procedures are also coordinated in-network. 

SCCIPA physicians all have access to software that enables data sharing, referrals, and proactive identification of at-risk patients. The association also has a team of case managers and nurses that collaborate with the physicians with the goal of improving health outcomes and decreasing healthcare costs. 

SCCIPA prides itself on high-quality, patient-focused care. In 2015, the Integrated Healthcare Association awarded SCCIPA its Ronald P. Bangasser, MD, Memorial Award for Quality Improvement due to its advances in clinical quality, patient experience, and use of information technology.

When your employees enroll in a health plan from one of Canopy Health’s carrier partners, they can select a SCCIPA doctor as their PCP, which automatically enrolls them in the Canopy Health alliance. In addition to the hundreds of physicians practicing within the Santa Clara County IPA, Canopy Health members have access to our broader alliance of 5,000 physicians and 18 hospitals in eight Bay Area counties. This gives Canopy Health members the option of treating with a PCP or specialist wherever they live, work, or play.
